All we need is the information listed below.1. Venue name2. Date and time of your party3. Your name4. Email address or fax number to which the list must be sent5. When we should send the completed list to you (day and time)6. The name of the party7. The name of the list (putting 'Clubplanet' in there somewhere is recommended)8. Normal cost without list9. Guestlist details -- how much is admission when on the list and under what conditions (such as before which times, if there is more than one cutoff i.e. $5 before 10 pm, $10 before 11pm, ladies' discount, dress code, etc.)10. Time at which the list closes at the door of the venue11. When signup for the list closes (day and time)12. Does the list have an expiration date (not meaning a day of the week, but a certain number of weeks from now, for instance)? If so, when does it expire?13.
